MEDICINE OR ART? During the pre-operatory consultation, Dr. Koray will perform a scientific analysis of the characteristics of the hair and the patient’s alopecia to create the best plan for the imminent and possible future operations. A study of the facial characteristics will be done with the goal of creating the most suitable aspect according to his necessities and personal features.
Numerous psychology and neurological studies are demonstrating how one person’s hairline has a deep influence on facial identification and, as consequence, on the perception other individual have. Because of this it is extremely important to give value to the aesthetical features of the patient creating the most balanced hairline design.
During the surgery the each extracted graft is carefully checked in order to be selected and distributed over the balding areas according to precise technical criteria. Through this great attention to detail in every phase of the surgery we can offer the most natural reconstruction of the patient’s hairline.
THE -DES- MANUAL FUE EXTRACTION TECHNIQUE Generally speaking, FUE extraction consists of the “one by one” extraction of the little organs that produce hair: the hair follicles. They are extracted from the back and lateral areas of the patient’s scalp (the “donor area”), with the aid of a medical tool called a “punch”, a cylindrical blade having a hollow central part whose diameter may vary from 0.6 to 1.00 mm. Then, they are artistically implanted in the balding parts, which are getting the name of “recipient area”, to create a new frontline and to improve the density of the existing hair. Punches can be manual or electric (micromotor). Choosing one extraction method or the other depend on the preferences of the physician but we believe manual FUE is offering the best overall performance.
The main advantages of the FUE procedure in comparison to FUT (also known as “strip”), the other popular hair transplant technique, are:
• No linear scarring. Patients are able to wear their hair very short or shaved without limitation.
• FUE surgeries are far less invasive than “strip”, with better and faster healing.
• Differently from the “strip”, FUE does not increase the overall scalp tension.
• FUE homogenize the general hair appearance. As a consequence by increasing the hair density over the recipient and decreasing it over the donor area. This way more aesthetical final outcomes are obtained, with the “illusion” of a higher recipient density.
At the ASMED clinic, the Manual Punch is exclusively used to extract the follicular units from the patient’s donor area. The instrument is primarily composed of two parts; a metal handle to which a cylindrical blade is applied, designed by Dr. Koray and having a hollow central part with a diameter between 0.6 and 0.8 mm. Graft extraction is performed through the DES Manual FUE, an exclusive FUE technique created and patented by Dr. Koray Erdogan.
The main features of the DES Manual FUE are:
• It is performed in a sitting position with local aesthesia, instead of a prone position. The donor area’s skin is stretched manually and this allows for a more precise reach of the follicles by the extractor.
• During the surgery, patients are able to read, watch movies and have a conversation with a personal assistant.
• Allows for better hand sensibility by the FUE extractor once the Manual Punch penetrates the skin, which is a fundamental feature in order to adjust to the follicle’s anatomy.
• The penetration of the punch is always perpendicular, rather than oblique, then changing direction under the skin. This results on minimal trauma for the operated part and, as consequence, minor scarring over the donor area.
• This is leading to important benefits in the perspective of future surgeries, as well as visible benefits for the appearance of the donor area in case of the patient, in the future, desires to shave or cut his hair very short.
• Penetration of the skin is done through manual oscillation. Therefore, the hair grafts won’t be heated by the high-speed rotation of some models of powered systems.
• Provides high quality FUE grafts and consequently, the attainment of highly satisfying results for a broader number of patients.
• Allows for an accurate donor area management, with a homogeneous extraction and the reach of the parietal scalp (lateral sides). Therefore, a larger number of hair grafts can be extracted in the event of future operations.
During the whole extraction phase, an assistant operator will be checking and counting each graft by using a patented software. This is done with the purpose of creating personal statistics to be given to the patient as well as to be used by the clinic for future references.
The tiny openings that will host the extracted grafts will be created using exclusively the refined “Lateral Slit”, the coronal incision technique. The procedure allows to reply the correct hair direction, to optimize the hair transplant density and to efficiently work in between the native hair by respecting the existing hair follicles and offering a more homogeneous and natural outcome.
